How to Prepare For Your Appointment
Digital documentation makes appointment prep easier than ever.
DOCUMENTATION
Prior to appointment, patients will receive consents and assessments via e-mail or text.
ALL documentation must be completed prior to appointment.
TELEHEALTH
Many appointments can be completed via telehealth, including follow-ups urgent care. Patients will receive links via email and text prior to appointment.
CANCELLATIONS
No-shows and last minute cancellations are subject to a $75 fee. Please notify us within 24 hours to avoid fees. After three last minute cancellations or no-shows, you may be terminated as a patient.
